When the "Report" is clicked it sends a game report to the Arcade admin section for reports, but there is no reason for why the game was reported and it does not notify anyone that a game report has been submitted. The "Report" link changes to "Game Error Reported". This can easily be missed if you have hundreds of games on your website.
Can it be made that (1) when a game error report is submitted that it allows the member who is reporting the game to input a reason why they are submitting the report and (2) either send a notification to staff members or post a topic in a pre-selected forum so that staff can easily identify the report and take action to either fix or delete the game ?

With the amount of games failing for one reason or another these ideas would be a good addition to the mod.
Yes that would be a very good option. That the whole thing is posted. So the user and the admin see the created thread with the game and the error what doesn't work.
Reporting a game issue already allows the user to state the reason. This information is available on the list by clicking or tapping the appropriate column will display a small popup window containing what was reported.
I can implement a PM option to all admin and arcade moderators.
